We partner to provide support to older people, those with a disability or people who are facing a social crisis.What's on offer:Based at our Malaga location – Servicing the North Metro regionFull time role (8am – 4pm, Monday to Friday), $47.50 per hour + Salary Packaging options and SuperannuationFriendly and supportive team working with a positive workplace culture Would consider a ‘job share’ arrangement for the right candidate/sThe program: The ‘care finder’ program is designed to assist Senior Australians in accessing and understanding the aged care system. care finders will provide specialist assistance to help connect participants to relevant programs and supports in the community.Participants in the program may require intensive support due to:Isolation or lack of a support person (e.g. carer, family or representative) to assist them in accessing aged care services via My Aged Care.Communication barriers, including limited literacy skillsDifficulties processing information to make decisionsResistance in engaging with aged care for any reason, where their safety may be at immediate risk, or they may end up in a crisis situation within the next 12 monthsPast experiences that mean they are hesitant to engage with aged care, institutions or government agencies Primary responsibilities:Outreach (visiting in home, engaging clients in different environments etc).Case Management, data and record keeping.Engagement and rapport building with potential client and other services.Supporting people to interact with My Aged Care portal. Support to explain and guide through the assessment process include providing support at the assessment.Access Care and Housing (including hoarding and squalor)Support to help people to find aged care services they need and to connect with other relevant supports in the community, include supporting people to:Understand the different types of aged care supports and services.Find and make an informed choice about providers/services. Work through income/means testing, if relevant and costs involved. Complete relevant service and health forms. Meet with providers to arrange services. Connect with other relevant supports in the community. What you will need:A kind, caring and respectful approach to service provisionSelf-motivated with the ability to work effectively in both a team environment and independently with minimal supervisionAbility to work with individuals from a range of social, financial and cultural backgroundsStrong communication and interpersonal skillsAppropriate qualifications or experience in aged care, human/community services or equivalentDeep understanding and knowledge of aged care and related servicesDemonstrated ability to exercise initiative and discretionCurrent national police check (not older than 6 months)WA Drivers LicenceBackground or qualifications in Social Work would be advantageous How to apply:Applications should include a resume and preferably a brief cover letter outlining your suitability for the role.
