We believe in creating opportunities so all Victorians have the choice to access essential primary health care services.An opportunity for part time 0.6FTE, this position is a fixed term role until 30/06/2024 based in the Omeo region.  Position Purpose: The RFDS Social Outreach Service is a program that aims to improve mental and physical health outcomes by supporting individuals and communities to improve their social connectedness and access to wellbeing supports.  The role requires a social, positive and curious individual who has the wellbeing of their community at the forefront of their actions.The Social Outreach Worker will liaise with existing agencies, collaborate and strengthen existing services/initiatives and attempt to address identified service gaps (within scope). They will work collaboratively with individuals, stakeholders and other agencies, and will adapt the response according to the identified needs.Community engagement will be essential to build trust and rapport and engage hard to reach individuals in community.This position is fixed term until 30 June, 2024, with the possibility of extension.The Social Outreach Worker will participate in regular case management, individual and group supervision and comply with all RFDS policies and procedures.This role involves regular travel across the delivery catchment area to facilitate service delivery. Key AccountabilitiesProvide wellbeing support engagements to individuals that may include psychosocial support, psychoeducation, service referrals, risk management and escalation.Provide follow up engagement and wellbeing support.Provide service users with a range of information and support to meet their individual needs and goals.Proactively engage with local communities and key stakeholders to build trust and rapport and to promote service access pathways.Develop community knowledge of relevant services and enable seamless referrals and handover.Refer within the established stepped care framework for the region.Maintain confidentiality.Travel to isolated communities in your designated catchment.Some weekend event work may be required.Key Selection CriteriaEssential:A positive attitude and a genuine desire to make a differenceQualifications (or significant work experience either paid or voluntary) in one of the following:Community developmentSupport coordination/Case ManagementEducation or Health promotionCounselling/ Social work/Mental healthOther related qualifications may be consideredStrong understanding of the local area and its strengths and challengesDemonstrated experience in conducting community engagement activitiesDemonstrated experience working with people in a role that requires good listening and rapport building skills.Ability to build relationships and collaborate with local service providersDemonstrated capacity to quickly learn and following detailed proceduresAbility to work autonomously and as part of a teamCurrent Working with Children CheckSatisfactory National Police CheckHold a current Victorian Drivers License and a reliable car Desirable:Experience (either work or lived experience) providing wellbeing supportCommunity minded with demonstrated experience in a volunteer capacityThis is a rare opportunity to work for an iconic Australian organisation that promotes a supportive and inclusive working culture.
